A one-page Illinois Living Will to declare your desire that if suffering from a terminal condition you prefer death delaying medical procedures be withheld. This document is statute-based and directs an attending physician to allow you to die naturally if your death is imminent. Otherwise, family members make the decision whether to prolong treatment regardless of your intent.

Illinois law does not require the document to be notarized, but this App includes provisions for a notary to prove your signature and that two witnesses were genuine.
